Case Management Services
Program
Description
The
purpose of the
Case Management Services Program is to provide goal-oriented and
individualized
supports focusing on improved self-sufficiency through assessment,
planning,
linkage, advocacy, coordination and monitoring activities and
supportive
counseling and crisis intervention when necessary. Case management services are available 8:30 a.m. to 5:00 p.m. Monday through Friday. Other times are available on an as-needed basis. Case Managers are based at both our Greer and Simpsonville Clinic locations. After-hours emergencies are managed by the designated on-call staff and can be reached by calling Crisis Line.
There are no specific diagnostic criteria requirements for admission into the Case Management Services Program. However, the majority of those who can benefit from this level of care will have a major psychiatric illness with functional deficits. Persons may enter the Case Management Services Program as the result of needs identified in the Initial Clinical Assessment; On the recommendation of another Behavioral Health Organization that has completed an Initial Clinical assessment; By recommendation of their therapist or physician; As a result of a re-assessment or other change in status; As identified by the Center’s authorized Hospital Liaison; Or by request of the person served or his or her family member.
Philosophy
of the
Program
The
philosophy of
the Case Management Program is to provide the appropriate level of
services
necessary to enhance community tenure and to empower the consumer in
accessing
and utilizing available community resources.
The Case Management Service
Program seeks to achieve maximum
levels of
functional potential and increased social connectedness based on the
choices of
the persons served. Case Management Services are carried out in
partnership
with the persons served and are highly individualized. Goals
of the Program
- Support and develop communitytenure.
- Assist persons served in
findingdecent and affordable housing.
- Encourage and supportemployment.
- Provide
linkages to
primary health care and other community services as needed.
- Enhance quality of life through skills training related to problem solving, seeking out healthy recreational activities, and in development of meaningful relationships with family and friends.

Populations
For
individuals who
are Deaf or Hard of Hearing, services may be provided in cooperation
with the
Division of Services for the Death and Hard of Hearing.Persons with a co-occurring substance abuse disorders may be served in cooperation with the local Drug and Alcohol Abuse Commission.
All service locations are Handicap Accessible.
Interpreter services are available for non-English speaking persons.
Facilities
and
Resources
Case
Management
Services will be provided by a variety of Professional and
Para-professional
staff to include, Physicians, Nurses, Mental Health Professionals,
Clinical
Counselors and Therapeutic Assistants.
Caseload sizes will vary,
from very small (10 to 12 consumers
per staff
member) to average, (50 to 60 consumers per staff member), depending on
the
needs and preferences of the persons served.Staff members working in the Case Management Program will be able to demonstrate knowledge of health care, social services, employment, housing, recreational opportunities and other services and systems available in the community. Services are provided in locations that meet the needs of the persons served.
Staff members working in the Case Management Program are certified in Basic First Aid and Adult CPR.
